In order for our us to serve our students more comprehensively, we request that if your child(ren) has health issues, please PRINT and complete the appropriate forms below. A new medical plan must be submitted each school year.

Medication at School Summary- Parent Letter

Medication Administration Form - Must be printed and submitted to the school.

Self Administration Form - this form needs to be signed and returned to the school for any student self carrying a medication. Over the counter medications are only allowed to be self carried at the high school level. WBSD still requires a contract to self carry and medication administration form filled out by the doctor and turned into the school every school year.
