• Work Permit
     
    Work Permit Procedure:
    1. Pick up a work permit in the high school Principal's Office, or read this WORK PERMIT INFORMATION & WORK PERMIT FORMS and then print the Work Permit that corresponds to your age.  Form CA-7 is for students that are 16 & 17. Form CA-6 is for students that are UNDER 16.
    2. Complete the student portion of the Work Permit.
    3. Take the work permit to the Employer, who will complete the Employer portion of the Work Permit.
    4. Bring the completed Work Permit to Mrs. Weitz In the Principals' Office, who will approve the Work Permit for you.  The teenager must also bring at least one piece of identification which shows birth date with the completed work permit before we can approve the request.
    5. Take the completed Work Permit to your employer, who will keep it on file. 
    6. If you are printing a work permit to complete, print BOTH the WORK PERMIT INFORMATION & WORK PERMIT FORMS that corresponds to your age.  Form CA-7 is for students that are 16 & 17.  Form CA-6 is for students that are UNDER 16.
    The teenager must bring at least one piece of identification with the completed work permit before we can approve the request.
     
    Work Permit forms must be fully completed by both the teenager and the employer before the High School can issue a completed, approved Work Permit.
